Application Checklist and Forms

1. Request admission information

2. Complete and submit the online Preliminary Application with the application fee of $70 by Thursday, December 10, 2009. Filing the Preliminary Application online allows you secure access to check the status of the application throughout the admission process via the universityprep.org website, including the admission decision on March 19, 2010. Alternately, you can print and mail this Preliminary Application with the $70 application fee to: Admission Office, University Prep, 8000 25th Avenue NE, Seattle, WA  98115.

After we receive your Preliminary Application, we will assign your child a school visit date. All visits begin at 7:45 a.m. and applicants are interviewed during their campus visit. Parents are invited to tour the school on their child's visit date. 

3. Register your child for the Independent School Entrance Exam (ISEE). Look here for additional ISEE information and a list of local test sites and dates. Applicants should take the ISEE by the end of January 2010.

4. Complete the Parent Statement and Student Essay and e-mail them to admissionoffice@universityprep.org.

5. Print and sign the Request for Records form and submit it to your child's current school.

6. Print two Teacher Evaluation forms and forward them to your child's current English and math teachers. If your child has the same teacher for both subjects, please ask another academic teacher or last year’s teacher to complete the form. Please request that they complete the forms and mail them directly to: Admission Office, University Prep, 8000 25th Avenue NE, Seattle, WA 98115 or fax them to 206.525.5320. If you prefer, you can e-mail your child's English and math teachers requesting that they complete this Word version of the Teacher Evaluation form. 

We allow schools until the end of January 2010 to submit student records/transcripts and teacher evaluations.

All application materials are due in the Admission Office by Thursday, January 14, 2010.
